The Laurels’ Admissions Team is available to accept your referral 24 hours a day, 7 days a week. When you call The Laurels, we respond quickly because we have adopted a communications system to be there when you want us, where you need us.

 

The Laurel Process

 

First, call the Director of Marketing or Admissions Coordinator. Once you’ve contacted us about accepting the patient, we take an interdisciplinary approach with our medical team before responding. We want to ensure that The Laurels can meet the physical, social and emotional needs of each potential guest. We will not accept patients if we cannot meet their needs. Please refer to the Medically Complex Programs of this website to determine whether our comprehensive medical services meet your patient’s needs. Once we have a response to your inquiry, we will contact you to arrange a tour for the family and guest, or to schedule the admission.

 

Helpful Admission Hints

 

The following are a few guidelines to help simplify the admission process to The Laurels:

  • Ensure physician and pre-admission screening certification is completed.

  • We will need a physical exam to be conducted, including a chest X-ray and TB testing, within five days prior to admission to a skilled nursing facility.

  • We hope that the patient’s payment source is determined in advance. The application for Medicaid should be completed prior to admission.

  • Please remember that acceptance for admission to The Laurels is contingent on our ability to meet the appropriate care needs of the patient.

  • We will need copies of the following from the patient:

  • Medicare Card

  • Insurance Card

  • Medicaid Card

  • Social Security Card

  • Durable Power of Attorney/Guardianship Documentation

  • Living Will
