Patient Description

 

The Laurels have always provided highly effective Medicare Part A programs for patients who require rehabilitation. Through our Prepare Program, The Laurels’ in-house rehabilitation staff meets the needs of Medicaid-covered individuals who live at home with the assistance of the Medicaid Home Waiver program, yet require a short-term inpatient restorative program. The Prepare Program is able to provide inpatient restorative services whether an individual is eligible for a Part-A covered stay, Part B-covered therapy, or has no coverage beyond Medicaid.

 

Program Design

 

The same in-house rehabilitation staff that provides Part A-covered rehabilitation plays an integral role in the care of individuals in the Prepare Program. The Laurels’ Rehabilitation Services Director and in-house rehabilitation staff oversee the delivery of physician-prescribed therapy and restorative programs. Our nine different restorative programs can be tailored to allow Medicaid Home Waiver participants to achieve and maintain the highest physical, mental and psychological well-being.

 

The Prepare Program focuses on the key activities of daily living that will promote safety and well-being for individuals who plan to live independently. These daily living skills include mobility and ambulation, the use of orthotics or prosthetics, dressing and grooming, meal preparation, effective swallowing techniques, proper nutrition and hydration and communication.
