Cardiac Recovery Program

 

Patient Diagnosis

 

The Laurels’ Cardiac Recovery Program is appropriate for individuals with diagnoses such as congestive heart failure, myocardial infarctions and coronary artery bypass surgery requiring short- or long-term rehabilitation.

 

Program Design

 

Our Cardiac Recovery Program is designed for patients needing:

  • Respiratory care

  • Peripheral I.V. antibiotics

  • Heparin or Coumadin management

  • Vital sign monitoring

  • Treatment of complicating conditions such as renal disease or diabetes

  • Incision care

  • Physical therapist directed cardiovascular conditioning

  • Patient/family education

An interdisciplinary team will meet routinely to establish treatment goals and review progress.  The team will consist of the patient’s physician, case manager, registered nurse, in-house physical and occupational therapists, registered dietitian, social worker and consulting respiratory therapist, as well as other pertinent disciplines.

 

Program Protocol

 

The Laurels’ Cardiac Recovery Program is appropriate for adult patients who have:

  • An established diagnosis

  • Completed any invasive procedures

  • Stabilized in their medical condition (e.g. stable cardiac status, no excessive bleeding) or

  • A prognosis of recovery and discharge to a lower level of care

Patients remain in the Cardiac Recovery Program until established goals are met or until transfer to a lower level of care can occur.

 

Attending and/or consulting physicians are welcome to follow their patients participating in our Medically Complex Programs. If that is not feasible, a member of The Laurels’ medical staff will attend to the patient.
