Medically Complex Programs

 

The Laurels’ Medically Complex Programs (see list) offer comprehensive care provided by our interdisciplinary team for patients requiring short-term medical and/or rehabilitation interventions. These programs serve as an inpatient alternative to an acute care hospital setting or continued hospitalization when home or outpatient care is not practical. The Laurels’ medical and rehabilitative services provide high quality medical treatment that is extremely cost-effective when compared to intensive home health services or hospitalization.

 

An Interdisciplinary Team Approach

 

Each Medically Complex Program utilizes an interdisciplinary team of experienced clinicians to achieve specific patient outcomes. This team meets upon a patient’s admission and then at least biweekly thereafter to assess, plan and review patient goals, treatment and progress. A licensed therapist or registered nurse case manages medically complex patients throughout their stay. We have found that an interdisciplinary team approach, involving the physician, nurse, therapist, dietitian and social worker to address the medically complex population with multiple problems, results in the most positive outcomes.

The Laurels’ Medically Complex Programs are further differentiated by our experienced in-house therapy staff that provides consistent care to our guests, as well as training for family members when necessary. Program Protocol

 

The Laurels’ Medically Complex Programs are appropriate for adult patients who:

  • Have an acute event as a result of an illness, injury or exacerbation of a disease process

  • Have an established diagnosis and course of treatment

  • Do not require extensive diagnostic and/or invasive procedures

  • Have a prognosis of recovery to allow discharge to a lower level of care

  • Patients participate in The Laurels’ Medically Complex Programs until established goals are met or until transfer to a lower level of care can occur.

Physician Participation

 

Attending primary care physicians and/or consultants are welcome to follow their patients at The Laurels. If that is not feasible, a member of The Laurels’ medical staff will attend to the patient.
